If benefits are to be derived from athletics, the highest standard of conduct and sportsmanship must be maintained. The conduct and sportsmanship of the players, coaches, officials and others associated with the game should be exemplary. Everyone participating should conduct himself/herself so that he/she is a credit to the sport.

This program exists to create opportunities for athletes to participate in Iowa Rush that might not be possible due to financial reasons.

This form must be completed each season in order to be considered eligible for Financial Aid. Fill out the application as completely as possible. Your privacy will be carefully protected.

The purpose of the Iowa Soccer Association Risk Management Plan is to communicate methods of minimizing/preventing risk to players, volunteers, paid personnel, spectators, clubs and leagues. This Plan does not provide professional legal or tax advice and may not be relied upon for that purpose. While this program may require time and effort, there is no higher priority than the protection of the soccer players and volunteers within our organization.

This request form must be completed in its entirety before it will be reviewed by Iowa Rush South board members to determine if a refund or partial refund is approved or possible based on what the registration was for, the refund policy stated during that registration, the timing of the refund request to when the soccer program begins, and any circumstances beyond control such as injury or moving from the Pella location.

The Medical Release Form MUST be signed by parent/guardian and include copy of front/back of insurance card.
